The central defender has struggled to nail down a spot in the starting lineup during his spell at Stamford Bridge and had an underwhelming loan spell at Aston Villa in the second half of last season.
However, Disasi is capable of being a valuable player to a side that is at the lower end of the Premier League table, so a move to West Ham could be a good option for all parties involved.
Disasi has had a number of potential suitors in the ongoing transfer window, but a move to the London Stadium does look likely on deadline day.
The only potential stumbling block in the deal could be the financial aspect, but both clubs are working towards resolving that.

Disasi is definitely one of the players who is likely to leave Chelsea, given that he is not a part of the first team squad.
However, it might prove to be a complicated deal as Chelsea value the Frenchman at ÂŁ30m ($40m), which might be a steep price for a club like West Ham United.
Despite the complications of this deal, it remains one worth keeping an eye during the last day of the transfer window.
